ATTORNEY SERVICE PROGRAM

DISABILITY CONSULTATION SERVICES:

Proactive Primary Care Services, Inc, founded by Dr. Vanessa M. Dazio, OTD, OTR/L, will consult with your client, to identify disabling factors and the effect of such factors on your client’s life.

We will include a detailed analysis about the medical illnesses, injuries or conditions the client has and how such conditions affect the client’s daily life performance.

We will then develop a comprehensive written summary, in a structured format, that provides specific information about your client’s functional impairments, daily life limitations, participation restrictions in the home, work and community environments.

The analysis and summary report will carefully and thoroughly explain how such impairments affects the client’s ability to complete tasks and activities of daily living and affects his or her ability to work and actively participate in valued life functions.

The final report will serve to provide an even clearer explanation of the depth of disability experienced by your claimant.

This service is offered to expedite the initial determination and/or the appeal process.

ABOUT OCCUPATIONAL THERAPY

Occupational therapy practitioners are considered “experts” in function. As such, they work with individuals who have or are at risk of developing an illness, injury, disease, disorder, condition, impairment, disability, activity limitation, or inability to participate in valued life activities.

Occupational therapists identify functional losses in daily life, risks, challenges and barriers that affect health, wellness, safety and quality of life. If disabilities are determined, the occupational therapist will work to maximize function, health, safety and wellness.
